Initial Comments:
An off-site follow-up survey completed on July 22, 2022, found that Lifesong Hospice and Palliative Care, had corrected the deficiencies cited under the requirements of 42 CFR, Part 418, Subparts A, C & D, Conditions of Participation: Hospice Care. The deficiencies were cited as the results of a state licensure and recertifcation Medicare survey completed on May 27, 2022.
